Orthopedics Record Request

Sano has partnered with Record Quest to ensure accurate and timely processing of medical record requests. Have your driver’s license or government ID ready. Please allow 48 business hours to process request.

Other Medical Records Requests

For vascular and vein or printable record requests, please complete our medical release form to request a copy of your medical records. Fax the completed form for 816-455-9125 or text it to 816-551-2339.
